Pathway to Wellbeing Scheme

The Pathway to Wellbeing Scheme can help support you to lead a healthier lifestyle and improve your wellbeing through access to a range of programmes and activities, with something for everyone! Whether you’re looking for ways to increase physical activity levels, eat more healthily or need extra support in reaching your goals the wellbeing scheme can work for you! We can help you get started with activities such as gardening, walking or cycling or get you set up with activities in leisure centres such as a gym programme, fitness classes, swimming or even an adult informal learning course to help you try out something new.

Yes! The scheme is available to all providing you are 16 years of age and live within the Reading Borough.To find out which programmes suit your needs look at the programme information below.

What programmes can anyone book onto?

  • Let's Get Moving- One to one sessions with a physical activity expert who will assess your current activity levels and work with you to find ways to increase your activity levels.

What programmes do I need to get a referral from my Doctors Surgery for?

  • Exercise Referral Scheme - This programme is for people with specific medical conditions such as Obesity, Cardiovascular disease ,Asthma ,Diabetes ,Depression and stress ,Arthritis ,COPD, Stroke ,MS who would require a referral from their Doctor to take part in supervised activities at a Reading Leisure Centres. The choice of activities includes, Health walks, Cycling, Circuit training, Chair based exercises and Aqua mobility
  • Eat 4 Health – An interactive 10 week weight loss programme for adults which included nutritional advice followed by a physical activity session.
  • Phase IV Cardiac Rehabilitation – These sessions follow on from a hospital based physical activity rehab programme and are specifically for people with Coronary Heart Disease who are recovering from Heart Attacks or Heart surgery.
  • Men’s Healthy Lifestyle Programme (through NHS Health Checks) - If you have received a men’s health check through your GP or workplace then your GP can recommend you to the Men’s Healthy Lifestyle Programme Where Men in their 40’s and 50’s have access to one to one physical activity support and can also take part in a new 12 week Full Body Service (FBS) email package covering a range of health and wellbeing topics suited to the individual.
